Testosterone and changes in appearance of the eyes.
A random comment on Reddit mentioned they personally had blepharitis as one of their issues due to menopause. That fits with my hypothesis about hormones as an important part of the immune system because it's primary cause is infection, so I searched Reddit for "trans blepharitis" and tripped across the above post about changes to the eye.
